We were looking into having it down for my daughter who has Cerebral Palsy in hopes of helping oxygen flow to the damaged part of her brain and helping her possibly. We were quoted a price of $3,000. I have had a friend who also pays about that $3,100 and has had it done 32 times for her son! She fundraised all the money and took out a second mortage. It didnt work well, its effects were temp. which is why they went back so many times

that's actually a difficult question. There are many parts to the surgery, such as anathesia, surgeon, medicine charges, lab tests, recovery room charges, etc.
The hospital bills the insurance company whatever they want. Each insurance company pays a set amount, no matter how much the hospital charges are.

My wife took 6 treatments in North Carolina. $60 dollars each. You can figure $1/minute. Some offices offer portable units that you can take home.
