President Abraham Lincoln most probably died from a subdural hematoma: brain swelling. Caused by heavy bleeding in the brain resulting from a single gunshot wound to the area Temporal bone behind his left ear with a .44 caliber bullet that traveled transversely through the brain and lodged behind his right eye.

The shot itself did enough brain damage to put the president into a coma, but not enough to kill him. The external bleeding was controlled so he did not bleed out. Therefore the continued bleeding inside his brain built up enough pressure that blood flow to the brain stopped and the patient died as his heart and lungs failed or the blood pressure in the brain could have damaged sensitive brain tissue sufficiently enough to destroy portions in control of his autonomic responses thus resulting in a failure of his heart and lungs.

Using a probe, Barnes located some fragments of Lincoln's skull and the ball lodged 6 inches (15 cm) inside his brain. Lincoln never regained consciousness and was officially pronounced dead at 7:22 a.m. April 15, 1865. There is some disagreement among historians as to Stanton's words after Lincoln died. All agree he began "Now he belongs to the..." with some stating he said "ages", while others believe he said "angels". After Lincoln's body was returned to the White House, his body was prepared for his lying in state in the East Room.”
