Confide in a friend or significant other that you have this problem and are serious about overcoming it. If they agree to help you, keep track of the times you are late and by how many minutes. Once a week meet with your helper, go over how many times you were late and the total number of minutes you were late. Then you must strip and take a spanking from your helper. One swat for every minute late. After the first session your punctuality will increase each week until your bad habit is broken.

Hi, this will help--make a list of at least three things you
need to get done (even the most mundane things) prior
to the scheduled thing. For example,
If you have a meeting somewhere for work or school or whatever
at 3 p.m. make this kind of list first, and do it:
1. Do laundry at noon.
2. Call about Friday night movie at 1 p.m.
3. Take out trash at 2 p.m.
4. Organize study papers at 2:30 p.m.
This should help you get ready for your scheduled appt and should
help you be on time.

Get a watch. Get a calendar and use it. Check your calendar daily to verify your schedule for the day. Set your clock forward 15 minutes.
Consider riding with a friend that's always punctual.
Get married. You're wife will nag you enough to get you where you need to be on time.
Otherwise, move back in with Mom...She got you this far, didn't she?

Conditioning does the trick for me sometimes. I'm always late too but that's because I consciously don't want to be early. The clock things will work.

If you have to be somewhere at three and it takes 15 minutes to get there leave 45 minutes early in case you need to stop for gas or something.

adjust your watch to an hour earlier (mine is advanced 24 hours but I'm still late! :P)
or try your best to discipline yourself. you might be losing focus or deprioritizing important events, or suffering from attention deficit disorder.
you may want to consult a doctor, or even a hypnotist.
