If it is dated 1803 then it would be a small silver coin called a half dime. If it is nickel size it must be a 1903. Early half dimes are very scarce, as for 1803 they only minted 37,850. If a half dime then it would be 16.5 mm and have a bust facing right on the obverse and weigh 1.3 grams. There is an eagle on the reverse. The first nickel is the shield nickel and it indeed is smaller than today's nickel. They were minted from 1866-1883. Has a shield on the obverse and on the reverse is a large 5 in the center with stars around it.

Not from 1803. The first 5 cent piece, or nickel, appeared in 1866.
