Generically, the term would be first gentleman. But since you're always referred to as President once you've held the office, Bill would still be referred to as "President Clinton." Technically. But there would be come confusion, so he would probably be "former President Clinton."

The United States has never had a female president, so we've never grappled with this bit of etiquette. Or, to put it another way, there's no precedent for that president.
That said, our first female president will be referred to as "Madam President." This is how female heads of state are referred to in other countries. We can only assume the same rules will apply here. Addressing the president's husband (if she's married) is a bit trickier.
If Hillary Clinton were elected president, we'd most likely still refer to Bill Clinton as either "Mr. President" or "former President Clinton." Even after they're out of office, past commanders in chief retain the title. Call it a perk of the job.
Of course, not every potential presidential candidate is married to a previous ruler of the free world. For example, if Britney Spears were elected to the office (God help us), we'd all address Kevin Federline as "First Gentleman." This is how Dan Mulhern, the spouse of the governor of Michigan, is referred to. Same thing with Jose Miguel Tuason Arroyo, husband of Philippine president Gloria Macapagal-Arroyo.

In the case of Bill, being a former president, I would say that if you're talking about him it would be First Gentleman. If you're talking to him it would probably be Mr President as is customary for a former president.

Ex Presidents are still refered to as Mr. President. So the first husband would still be Mr. President even if he weren't a former president.

IF Hillary Rottweiler Clinton were to become Pres., we wouldn't have to worry about what to call Slick Willie, because Hell would have frozen over. As for a reality based situation, most likely, "Mr. Whatever." This has been a question for so long,and nobody has come up with a good solution.

or
Mr. President (As First Lady's are sometimes addressed Mrs. President)
Former President W
or
William Jefferson Clinton.
Because technically he would be addressed by his former rank as president before addressed in a lesser unelected position.
But there is no (Presidential) precedent for it yet.
