Sounds like Orchitis - see a urologist
Symptoms of orchitis are similar to those of testicular torsion. These can include:
* ejaculation of blood
* hematuria (blood in the urine)
* pain
* visible swelling of a testicle or testicles
This condition must be diagnosed by a physician.

Orchitis can be related to epididymitis infection that has spread to the testicles. Orchitis is a not as common as epididymitis, but should not be taken lightly as loss of function (hypogonadism) can occur in the testicles.
A common cause for childhood orchitis is mumps.

In most cases where orchitis is caused by epididymitis, treatment is an oral antibiotic such as cefalexin or ciprofloxacin until infection clears up.
For viral infections, antibiotics are not recommended.
In both causes non-steroidal anti-inflammatory drugs such as naproxen or ibuprofen are recommended to relieve pain. Sometimes stronger pain medications in the opiate category, particularly hydrocodone, are called for and are frequently prescribed by experienced emergency room physicians.

well past time to go see the doctor unless he is starting puberty and one testicle has achieved adult size and the other hasn't started growing at all. But even then I think he needs a thorough hed to toe and all points in between checkup to ensure he is generally healthy and to see what is going on down there.

Ja speak to a doctor, that happened to me, it turned out to be a vein wrapped around the testicle and it would have made it die and I could loose it in the process.
I had to go for a small operation where they just burn the vein off.
This was more tahn 10 years ago. Everything is cool downstairs now ;)

It's normal for them to hang at different positions. If one is very noticeably bigger then you should go see a doctor.
