I've never seen this happen, but the issue has come up a few times in casual "what if?" conversation. The NHL rule mentioned in another answer, of course, applies. But ... if there is no goalie available in the building, the coach has to do the next-best thing: ask for a volunteer on his roster.
I remember when a Sabres' goalie got hurt, Mike Foligno said he was ready to put on the pads if a second-stringer got hurt.
Best story along these lines: the Sabres one time had a goalie leave the building because he was upset he didn't get to play. The team paged its public relations man, who had done some goaltending in practice, and tried to sign him to an amateur contract in order to put him on the bench as a backup. However, the lineup had been handed in at that point, so it was too late. The rule has been changed since that time (1977 or so) to allow someone to play if both goalies get hurt.

Goalie is the only position that can be filled by anybody when disaster strikes and the starter and back up both get injured. You should do a search on Lester Patrick, he played a Stanley Cup game once as a disaster back up, and I seem to remember on several occasions where somebody in the crowd dressed as either a bench back up or actually played. The nhl.com link is the applicable rule.
Please read the rule I have posted. It clearly states that if a team has no one left, they can pull someone from the crowd and dress him or her as the goalie, regular season or playoffs.

Only the dressed players are eligible to play in the game, so one in the press box wouldn't help. Some teams have a player dress as the goalie in this case, usually a defenseman.

i think they would put in the goaltending coach if possible. Lester Patrick is a good example, but that was in the day when there was no such thing as a backup goalie. Trainers or other players with experience in goal would likely be the most suitable alternative as teams would not sacrifice one of their better players.

It has happened during a game. I think in the old NAHL in the 70's with the Philadelphia Firebirds. Both goalies went down and they dressed the trainer Dave "Suddsie" Settlemyer.

Wow ummm, there would have to be a minor league goalie scratched that travels with the team, that must be mandatory, if not they might just call the game or postpone it until further notice

Regular season you have to dress someone on the bench. Playoffs the goalie who is in the stands can come in. He is on the roster.
