i need a parent release form in transportation for an athlete?

We transport Special Olympians to and from events and practices in personal vehicles. I need a release form to that effect.

I hav one saved somewhere that I use for the highschool athletes I coach when I'm taking them in my personal car for practice and trips. It asks for health coverage number, parental signature etc. If I locate it I will edit my response and give you more details.
