ok, so first off, tattoos of a spouses, bf/gf, friends name are a very, VERY bad idea. remember that they are permanant and that even if you think that you will be together forever there is a chance that you won't be and you don't want to have some ex's name ingrained on your skin forever.
get him something that you know will have special meaning to him. if he's a big football fan, get him that autographed jeresy he's been wanting. if he's a book nerd, get him the collectors edition of his favorite. get something that he will look at as you being so in-tuned to his wants and needs. it will come to you, just give it time.

not a girl here, but if my gf was thinking of a to the end of life gift for valentines day...? 3 months from now? he must be very special to you. listen and pay attention to HIS interests. Sports? Music? Jewelry may work for a girl. But if my lady worked for three months to find out my favorite player or musician of all time? If she could ask the right questions to get the info? If she cared enough to even-- care what floated my boat? That one thing that no one else... That one retro jersey.. that one limited album... that handmade stained glass (I still have it!).
Think. It's easy to get him something that YOU think he'll like and keep... it's a little harder to get him something that you KNOW he'll treasure. Leave YOU out of it. Talk to his parents if you have to. Talk Listen Observe... Melt his heart! Make him say: "OMFG! I can't believe it!" or "Gee... thanks."
Be subtle. Listen. Observe. It'll come to you. Don't do what others say. Follow your heart and your gut.

A nice picture of him or the two of you would be nice. Or how about an album filled with pictures of the two of you on dates, having fun, going places, etc.

Get him a piece of jewellry with your names engraved on it. A ring, an ID bracelet or a necklace. Or you could surprise him with a gift certificate for matching tattoos. They are permanent!
Good luck.
