Does anyone remember this fantasy computer game from the 90s'?

Question

It involved a single character trying to make it through various levels. As he completed certain tasks he would gain the ability to morph into various forms. These forms would have different abilities. There were only 6 or 8 of these various forms. One could fly and another could hold his breath under water. There was an area on the screen with weapons and things he picked up. There was also a life force meter.
I would just like to find this game. I think it had "morph" in the name.

If you know a name of the updated version please let me do too.
