A personnel file contains the following:

Offers of employment, promotion or transfer
Proof of company orientation
Proof of identity and work eligibility
Any professional/clinical licenses you have
Your resume & job description
Emplyoyee health information
Employee counseling statements
Performance reviews
Requests for sick time or vacation

There may be a few variations from this, but it's a good rule of thumb.

It all depends on the circumstance of how the desk got scratched. If it got scratched because you grabbed your computer monitor and threw it at your boss, that may get documented in your personnel file. If it was a deliberate destruction of property, more than likely yes. But if it was just an accident...it may not necessarily be documented.

Typically your personnel file has your personal information such as name, ssn, dob, emergency contact numbers, any disciplinary actions taken against you, your job title and job description, and promotions including salary information. Whether or not that information is disclosed to another potential employer depends on the state you reside in. For instance, most employers will only release your job title and dates of employment to other potential employers (to avoid getting sued), in case you are looking for another job. Since salary information is considered private information, most the time that will not be released to another party without your written consent.

You may want to check with your HR department to see what their company policies are. Even if a disciplinary action is listed in your file, they may only retain information within a certain time frame (my job only keeps them for 6 months to date)

It depends on your company and supervisor.

There really is no hard criteria. Most companies keep things like attendance reports, performance reviews, your personal information, emergency info, disciplinary reports, and depending on what type of company what some call observation reports (work area tidyness,appearance etc.) thing's that they see on a daily basis that may or may not be kept for review's or disciplinary issues.
