as someone has already said irreversible, i'm going to say tim roth's the war zone...people walked out of the cinema when i saw it. probably worth a look but thoroughly nasty and depressing subject matter.
i'd been wanting to see cannibal holocaust since i was a kid after seeing the crazy video cover at the vid shop. finally got to see it the other day and couldnt believe how crap it was. absolute shi*te. watch it to get it out of your system like i did, at least then you can have a new film to say is the worst film youve ever seen...

Final Destination III
This isn't your ordinary horrifying Freddy Krouger Horror. I've seen Saw's I and II. I've seen Freddy vs.Jason. The Ring I and II. And more, but none could compare to Final Destination III. it'll leave you scared lights off and on. You'll have nightmares for weeks. You'll be questioning life for years. There is never a calm break or moment, there are blood cuddling moments, GRUESOME deaths, and suspense every second. If you're ready to take on the greatest ride of your life then get Final Destination III. Odds of living and dying are 1 to 1. Remember that.
This will only be worth your time if you really are a true horror movie fan. Saw I is like a kiddy movie compared to this.
Texas Chainsaw was a real actual story that happened but this guy is right, oh and I'm serious don't watch this movie unless if you are even just a little jumpy.

I've heard that "Last House on the Left" and "Audition" fulfill the description of brutal and unrelenting and are both supposed to be excellent, but I haven't seen either one.
Last House on the Left was directed by Wes Craven. The following is the synopsis on Netflix: "[The movie is about] a pair of repulsive, sadistic escaped convicts kidnap, rape and murder two teenage girls. Looks like they picked the wrong teens to mess with, though! The girls' parents, not content to leave matters in the hands of the law, set out to exact an equally brutal revenge on the vicious murderers".
Audition is a Japanese movie about a guy who holds a phony audition and the ticked-off, would-be actress who gets revenge after his little prank. I saw a clip of this and it freaked me out. She looks like a little school girl bubbling over with childish glee at torturing the poor guy.
I've been trying to get up the nerve to watch both of these.

Battle Royale. It's a japanese movie basked on a book of the same name by Koushoun Takami. Forty 14 year olds put on an island and all given different weapons. Then they have to kill each other until only one is left (Think society if Japan had won world war II).

Dawn of the Dead is sick an scary.
28 Days Later is also a great movie.
The Texas Chainsaw Massacre is a classic.(the original only.)
House of a 1000 Corpses is similarto Texas Chainsaw Massacre.
The Devil's Rejects will also piss you off and entertain you.

The most grueling and disturbing movie I saw was The Texas Chainsaw Massacre: The Beginning. It was just disgustingly gorry and it surved no purpose. The plot sucked. So I don't think you should watch it. But it fit into your category of brutal and unrelenting.
