It is called a "check presenter". A "book" is what you have your order pad in to take the order. Two seperate things. Usually a check presenter will have a credit card logo on it somewhere and a book has the restaurant logo on it.

book. I work in a restaurant & its always "wheres my book" "do you have a book" "how come theres never any books around here" (upon hearing the last one, the manager busted out like ten new ones, just today, I swear!) Thats what we servers call it (in the past 3 rest's I've worked in)

I've always heard "order book". It's where I and my help always kept the orders until tallying them and dropping them off at the table.
