One way to see for yourself is to take apart the sentence:
“I was sure” is the main clause, but “I did well on the test” is also a clause (a clause is just a word group that contains a subject and a verb). This second clause is a subordinate clause that is subordinated (or put to a lower rank) by an understood “that”; i.e. the sentence is understood to be “I was sure THAT I did well on the test.”
But if we take the subordinate clause and just look at the subject and verb, you would have two options: “I did well on the test” or “I done well on the test.” I should be clear which version is correct.

Neither. The sentence itself is in the past tense (I was sure). The thing that, at the time you had done (well on the test) is now past perfect. So:
I was sure I had done well on the test. Like -
I AM sure I did well on the test.
I WAS sure I HAD DONE well on the test.
