Most of the time, the colors on the old and new jack will be the same, watch where they come off & put them back the same.
If it was installed by a professional, you should be able to match the wires on the jack with the ones in the wall.
If done by an amatuer, make sure they go back the way they came off.

The red and green wires coming out of the wall are the only ones that matter. They go to the middle two pins of the phone jack. The phone jack might have two, four or six connections depending on the type. If none of them are red/green you just have to look close to see which two come from the middle contacts. It doesn't matter if you get the wires backwards.

If you have only one line, you will only need the red and green wires. The other wires are for additional lines. As stated before the new jack will have markings on it or the package it came in to match it up. Just don't touch the wires together.

your new phone jack should include a wiring diagram. Just follow that and you should be good to go.
