I haven't downloaded one, I just keep myself logged into Yahoo Messenger and use their Calendar for appointment reminders, you can set it to remind you at two different increments before the appointment and you can also tell it to give you a pop-up window reminder and/or an emailed reminder.
I use it to remind me to pay certain bills on a set day every month and also to remind me to fill out my timecard every Friday at work.
Works great for me!
