one of my main questions is do you have to put a date and sign your name at the end of the entree

you don't have to but it helps if you plan on reading through it agin to remember.

I would put a date on the top just cause it makes more sense that way and a diary entry is normally written all on the same day or you'd probubly start a new day. I've read some books that are formatted like diaries and they normally say some something like love, blahblah at the end of thier entry but thats up to you. I would put date and signature just to be safe. :)

That's optional. You can make it however you want... You'd usually start with Dear Diary/Journal...and then write whatever you want. You can put the date in the top right corner and end by signing your name, like you said. Whatever works best for you.

I've never heard of an academic format for journaling. I've written in one every day since childhood, usually just putting the date on the top right of the page. You can start with dear diary, or just start w/ today..........., or whatever you feel like writing about.
