The last name is Bahorik.
My other daughter's names are Kaitlyn Paige and Elizabeth Morgan. They are 3 y/o twins.

Alexandra Marie Bahorik. Since her first name is rather long try using a shorter middle name. Alexandra Rae, Alexandra Leigh, Alexandra Evian Bahorik, Alexandra Emily Bahorik
