Not as bad as you would think. The piercing is done and over with in seconds and that itself it not bad. The area will be sore for a few days to a week at most and chewing/smiling might be awkward for a while. Just relax and take deep breaths.
As long as you go to an experienced piercer, get it done right and keep the area clean and well taken care of you'll be fine.
If you get an infection or experience embedding of the jewelry or get hit in the mouth, then yeah, you'll be in pain.
As a person who does piercings and has 26 herself, you shouldn't freak out

I had my labret done and it was a strange one, compared to the rest. He clamped my lip and pulled it down. Starting from the inside of the lip he started pushing the needle through. This wasn't se bad, but when it hit the skin it got a little stuck and he had to change his position slightly to get it through the rest of the way.

it don't hurt bad at all. I had my boyfriend pierce my lip just a few days ago with out numbing it and i didn't flinch or anything.

Nope... That depends on how much pain you can handel... It felt like nothing the 1st time then the 2nd time OMG I wanted to cry... All that for nothing... now I just have 2 more holes on my face =o(

The piercing itself don't "hurt" its the swelling that sucks!!!

depends on your tolerance for pain. Just get it done its not that bad I dont have mine done but I have my belly button peirced and it didnt hurt at all.
