I dont get the ending. So the girl escapes from being a hooker, she goes to the hospital and while she is taking the elevator she meets up with that guy, that acts like a doctor, i dont know his name
Who is that guy and what does he do?

The man is Josh Hartnett's character from the beginning of the movie, but with a beard and dressed as a doctor.
He's a hit man (if you recall, the woman in the red dress at the begining of the movie paid him to kill her) and it's assumed that the prostitutes of Sin City paid him to kill the blue-eyed girl for ratting them out.
The blue-eyed girl knows who he is. She says goodbye to her mom one final time and shuts off her cell so Josh's character can do his job...

The girl made a mistake by calling her mom all the time. The hooker/crime ring/police force told her to not tell anyone anything or call anyone at all, but she still retained the close tie to her real family instead of just sticking to her hooker family. I guess most people either choose their family or prostitution police rings, you can't really have both. The guy is just a killer basically, and that's pretty much all that's important, he has to kill her because she talked to her mom and she was told not to numerous times.

Becky, the hooker (played by Alexis Bledel), was basically a "snitch" and she was found out by Gail (played by Rosario Dawson). The man in the elevator was a contract killer (known in the credits as the Salesman) hired to kill her (presumably for her snitching).

Josh Hartnett, who killed the girl in the beginning of the movie (he's also the first voice-over you hear) is the hired killer. I don't think he kills the girl right off the bat, when they show the first scenes he's scene with the girl a couple of times before they're on the rooftop, so methinks he inspires trust in them and then goes for the kill.
