You should avoid sites and services that require you to sign up for some monthly subscription. Once you start letting someone bill you monthly, it is hard to cancel.
I use a service called Phone Sherpa for my ringtones. I can upload any music file on my computer to their ringtone editor and make my own custom ringtone. I pick my favorite part of the song, and they send it to me in a text message. It is also super cheap and no monthly billing crap. They give you a free credit when you create an account, and if you refer friends, you can earn more. Start with your carrier's website - there should be links there to ringtones you can download. You pay a small one-time fee for each ringtone select. You download the ringtone to your phone and it's yours!
There are some sights were you "subscribe" and pay a monthly fee for a certain number of downloads per month. If you put "ringtone" into Google, you'll find a bunch of sites to review.
Make sure you know if your phone will support Hi-Fi or not - if not, you're basically paying for cheap midi song knock-offs. Also, if you're interested in downloading FREE ringtones, you could try About.com's cell phone section. They have some nice instrumental ringtones that are free.

For the interenet websites that offer many ringtones it is a monthly fee; if you are downloading from your phones internet shop it is a one time fee. You get more ringtones from the interenet site but how many ringtones do you really need? Haha

I just got one for the first time a couple of days ago. There were sites that sold them, but it seemed easiest going to my carrier. (Verizon Wireless.) It was a one-time $2.99 charge for one ringtone.

Usually you only have to pay once. But if you were to upgrade your phone, you would have to download it again, and therefore pay again.

If your downloading it from you phone, then its a one time pay deal. If your downloading it from the internet...dont know. I use musica real on my cell. It has real music, not digital stuff.
