Generally, write 'To' your manager and 'CC' to his/her manager and the Human Resources (Personnel) manager.
Start by expressing your desire for the company to succeed and that you enjoy working there.
List why you are an exceptional employee (accomplishments). Did you save the company money? Increased sales? Reduced waste? Handled a difficult customer? Met or beat deadlines? Worked extra hours on an emergency basis? That sort of stuff.
After that, transition into something like, 'because of my exceptional performance, I believe that I deserve an increase in my salary of $X,XXX per year (or $X.XX per hour)'.
Thank your manager for reading your letter and ask for a specific time to talk to him/her. For example,
"I'd appreciate the opportunity to talk to you about this by the end of next week.
Thank you."
