Because it's a natural plant extract, and does not contain harsh or man-made chemicals like what is found in modern body sprays and perfumes.
People who are into an "all-natural" lifestyle find that patchouli oil is a fragrance that conforms to their ideals about health & the environment.
I personally don't find the smell pleasant, but... that's why people wear it. :-)

Hippies that want to cover up the smell of pot. I hate the smell of patchouli. Reminds me of people in college who don't bathe.

I wear it every once in a while simply because I like the smell....but I have been called a dirty hippie, so that works too ;)
