If it's said with true Empathy and not just empty words then it means your pain is my pain. My thoughts, feelings, (heart) goes out to your mental and/or physical anguish.
When fact that we're all human and suffer the same, circumstances may differ, but the pain is the same, can really be seen and felt, and this is Empathy.
When Empathy exists in us, the heart has to go out to those who suffer. It has no choice, cuz the "other" is myself, it's my pain too.
It's the heart's nature to reach out, help, share, heal.
