How is identity formed? According to race? Sorry, I don't know what you are asking. In America, you are born and issued a birth certificate. You use this to get other forms of identification, such as a driver's license or a passport. There is no difference between races and I'm pretty sure that no acceptable form of identification in the US has any mention of race.

if you mean social identity, its primarily by job function, secondarily by personal attitude toward others. Race is not really a consideration although some racial bias still exists among the less educated (regardless of the racial orientation of the bigot)
