U.S. dollars can be exchanged at Cuban exchange offices called Cadeca (Casa de cambio). The exchange rate isn't favourable. U.S. dollars aren't accepted in Cuba, you'd have to exchange them for CUC (Cuban convertible pesos) or CUP (Cuban pesos, or moneda nacional, which is accepted only at basic establishments, most typically green markets and some state-run cafeterias).
Cash advances on Visa cards are possible, provided that your credit card isn't drawn on a U.S. bank. There are associated surcharges, though. The other problem is that ATMs are few and far between. You're better off bringing hard currency in cash (U.S. dollars will work, Euros or Canadian dollars would probably work even better).

if you are a US citizen you can not legally travel to Cuba. You risk a $25,000 fine, a year in jail and loss of your passport.
If you are not a US citizen, why would you go to Cuba? Would you have gone to Apartheid South Africa and supported that regime?
Please remember that THOUSANDS of Cubans have died trying to escape Cuba across the Florida Straits....if life is so good there under Castro, why are these people risking and losing their lives to leave?

no you cant spend us dollars in cuba. yes you can withdraw cash from atm's. the atm will automatically convert your dollars into cuban money.
