Missy-moo for a girl I know named Melissa.
Apparently as a toddler, Melissa was commonly called Missy by many of her relatives, which is normal enough. This got warped into the unfortunate nickname "Missy-Moo" because apparently Melissa liked the game peek a boo, A LOT. So to be cutesy, or what not, whenever Melissa would hide her face they'd say "Missy-Moo, where are you?" Unfortunately, it stuck, and has follwed her around ever since.

Booger, not from the movie, I really knew a kid with that nickname - was the cleanest kid alive - even his room was spotless - and he was PROUD of his nickname! He even has it emblazoned on his business card. Glad he isn't in the food business!

I didn't quite get this as a nickname when my husband first started calling me Fish Face but he assures me that it is a term of endearment. I still don't know if I believe him.

My old college roomate was named Gizzard
That started with him "looking like Gizmo" from Gremlins, so we called him Gizmo, then Giz, then Gizzard. Also my other roomate looked like Lenard Nemoy and his last name ended in "spaw" so that went from Spaw to Spock.
