This is a great free web site for generating an endless number of workouts no matter what your level or goal. http://www.swiminfo.com/swim-cgi/
I would spend at least two weeks focusing on the basics and getting your feel for the water back: lots of sculling, underwater streamlining, and drilling. Some Olympic level coaches choose to drill for a full month at the beginning of the season before they allow their athletes to swim normally. This is because it is easy to cheat on your strokes and build bad habits when you are out of shape. Plus stroke technique is so important to overall swimming. After a few weeks of drilling and sculling, start with workouts that build an aerobic base.

You've obviously done some workouts before but as a way to get back into it I would suggest a small pyramid of sorts. It'll break up the monotony and you'll bang out 1000 + yards in no time.

Go up and back...1 lap, 2 laps, all the way up to 10 and then back down..9, 8, 7 etc.

This website has great workouts. Our coaches have used several of these workouts with our club swimmers. Workout 24 is a standard.
My daughter, for 4 weeks now, has been using a pull buoy because of a knee problem. She is taking time off of all strokes and I credit the fact she has been swimming using only her upper body. Her speed and stamina have improved. Pull bouys are inexpensive and might be a good addition to your workout.

skinny-dipping in a public pool... you'll be thinking about the people trying to get a look at you and you'll forget that it has been so long since you swam that far.
