First of all, you must make an appointment with the Commissar of Party Affiliations. When you arrive, you will be asked to fill out form A$9876%$^5&^4 in triplicate. Then you must get the signatures of three Officially Authorized persons who have changed Party Affiliation, and who are out of prision. After you have completed all of the paperwork, you must get it notorized. Then you just go home for the 90 day waiting period. After that you simply appear in front of you Local Commitee of Concerned Citizens for approvial.

It depends what state you live in. Check with your local board of elections.
In many cases, you fill out a new voter registration card. Some places have special party affiliation change forms. In some states, you are able to just show up to vote at the primary of the party you wish to affiliate with. Your best bet is to check with the party you wish to switch to, the local board of elections, or some non-partisan organization like Rock the Vote.
