It seems that a lot of people here are a little confused. Here's the straight answer...

"Hip hop music" and "rap music" mean virtually the same thing. However, some people associate the term "rap" with the more mainstream variety of the genre (which nowadays often includes R&B or pop elements) and reserve the term "hip hop" for the variety of the genre that stays closer to the roots of hip hop music (i.e. - speaking on social and/or political issues, storytelling, bragging about one's lyrical skills while roasting another MC, etc.).

But to be really specific, hip hop itself is a culture. Rap music or hip hop music (specifically, MCing) is just one element of that culture. The other elements of the culture are graffiti, breakdancing (aka - breakin'), DJing and a more recently added element, beatboxing.

I'm going to assume that by "rap" and "hip-hop" you mean the genre of music and not "rapping" which would be the words that go along with the music.

Rap and hip hop in all actuality are the same thing. However, nowadays are separated into two separate genres. "Rap" music is a little harder than "hip-hop". Rap would be like DMX, Eazy-E, etc. while hip hop would be Luda, Nelly, etc.

When someone is rapping it would be when the rapper is talking. Like in the song Money Maker by Ludacris featuring Pharell, here is a verse that Ludacris raps.

Shake, shake, shake yo money maker
like you were shakin it for some paper
took yo mama 9 months to make ya
mine as well shake what yo mama gave ya.

Usually the chorus is the part that is sung, and that is the hip-hop part.
