Much like the others said, it is hard to explain without being able to show you. I also don't know how your teacher has been teaching it. I would say the first step is to memorize the sentence patterns. This goes through the 5 basic sentence patterns: http://college.hmco.com/devenglish/broughton/focus_florida/1e/students/diagramming_basic.html.
I would, however, as someone else advised, ask your teacher for extra help.

I don't know how to help you. I always found it easy, but I'd need graphics to illustrate it. Is there someone in your class who is good at it who could help you? I always found it kinda fun, actually. Sorry I can't sit down with you and explain it. Some people think it helps to use a highlighter and color all the verbs one color and all the nouns another, etc, before you start, but I never did that myself.

It's complicated to explain on this type of forum. I suggest you tell your teacher you are having problems and ask for extra help, during recess, after school, during study hall, or whenever your school schedule allows.

If that is not good for you, find out which of your friends understands it and ask them to come over for dinner and spend the evening having them explain it to you.
