it's going to be with my application whichhas my name on it, but i heard you shouldnt put your name on it so that the readers couldnt judge you? i dont know? should i put anything? or just the essay text?

No matter how you format it (I defer to the others on format--college was some time ago for me), definitely put your name on the page itself (say, in a header or footer). Imagine if, during the process of evaluation, when papers are all over the admission counselors' desks, that your single page gets mixed up in the shuffle--how will they know that piece of writing belongs to you?!

YES. Use a separate title page.

Put the name of the essay in the middle, and toward the bottom put your full name and the date you wrote it.

You need to double space it too.

Put it in 12-font and use Times New Roman.

I am in college right now and that is how all my professors want their essays. I PROMISE. :]

Yes, double space. Create a cover page and attach it.
