Also what constitutes a quarantine tank? Filter? Heating unit? Gravel/substrate? Etc?

It's strongly recommended to use one. Honestly, I never did, I've been lucky though I guess. One sick fish could wipe out your whole tank. Really, all you need is a basic tank, don't need to have gravel if you don't want it. Would recommend a filter and heater though. Could also use it as a sick/hospital tank as well if one gets sick, so the heater would be good.

I would also recommend getting a small mesh bag(find it at the pet store cheap) and putting some bio balls, or other bacteria type filter media, float that in your current filter/hang on the side for the established tank, and just leave it there. Then, whenever you use the QT/Hospital tank, put that bag into that filter or hang it from the side, and that tank will pretty much be instantly cycled.

You don't have to have anything fancy. The size depends on the size of your aquarium (and the size of the fish you get). I keep 2 one gallon aquariums as the fish I get tend to be quite small. Obviously if you have larger fish you'll need a bigger tank. I use an Azoo palm filter, and a small Hydor mini heater. To avoid have to run it constantly or go through cycling each time you want to add new fish, just keep an extra piece of filter media in the filter of your main tank. When you want to quarantine fish, put it in your quarantine tank's filter and let it run for about a day (to stabilize the temperature and stuff). Then you should be safe to put fish in it.

1-2 weeks is how long I quarantine. Some people suggest one month but I don't think that is really necessary.

Yes, otherwise you're risking all your other fish. A quarantine tank is only a small, temporary tank - yes it needs a heater and filter for sure. Gravel is probably not necessary since the fish won't be in there for very long.
