I'd say, the top 5, based on my Agnostic/Nihilistic/Egoist beliefs:
1. Commit suicide. Nothing is worse than the termination of self, because I believe in Egoism. Also you're punishing the people who love you by killing yourself.
2. Genocide. This is obvious, killing huge numbers of people is horrible and will turn the world against you. Great offenses against the environment also are up here.
3. Killing a family member. The only thing worse than killing a person is killing a person you're supposed to love.
4. Becoming a serial killer/serial rapist/pedophile.
5. Choosing to be a disrespectful person.

In my point of view the most horrible thing to do is to hate someone!I know that it may seem to be very ideal but it is true. You don't have to commit suicide in order to torment yourself ...you don't have to kill a family member to torment yourself or those who love and care...all what you should do is to hate someone ,after that see how it is very hard and tormenting ,you won't be able to sleep good ..you won't be able to laugh or smile ..YOU WON'T BE ABLE TO LIVE!

5) Pretending to be morally superior when you are really morally bankrupt...Like priests molesting children and preaching about love....
