The very best form is found in the yellow pages under "Attorneys." You simply cannot do it by yourself. Even if you could, do you know where to file it? What are the court costs? How to serve summons on the opposing party? Whether you even have grounds for the suit you plan? Doing it yourself and doing it wrong could leave you out in the cold if you had to then hire an attorney to do it right and find that the statute of limitations or some other legal bar makes you out of time for filing in the first place!

If you are talking about going to small claims court, the clerk would have the forms for you to fill out. Give them a call or visit - it would be with probate court in your county.
