It means to say exactly what you mean without adding to it or trying to be tactful. Often said when someone is being blunt or rude. But basically it means to get directly to the point instead of being polite about it.

It doesn't nessessarily mean being rude or blunt, but it does mean sticking to the topic and not rounding around the outside of the issue. Its direct and to the point.
