I worked in Plastic Surgery and, if you can live in your world where they kill whole familes I can live in my NIP/TUCK world.
I'd take both the 700lbs and the acne
THEN
I'd have gastric bypass and dermabrasion and skin grafts, then Pilates and excess skin removals, maybe learn kick boxing then I'd do some light botox, and have some work around the eyes and ears and come out looking a bit like this, & still save the family!

ill, take the acne. it would hurt and look ugly, but i could always wear a mask and put on soothing lotion. im to active to sit around all day and eat!

i would shoot myself. and if i had to chose acne cuz they have masks,you cant cover up a 700 lb. person!!
