What is the purpose of the little antenna on cell phones?

Do they actually get signals? If they do, then why don't I see people walking around with their little antennas up while they're talking?

When you raise the antenna you get a marginally better signal. It helps if you're in a low-coverage area.

I thought they were to improve reception and signal.
