It says your sim can become a werewolf. I know there is an antidote, but how does your sim actually become a werewolf? What do you have to do?

You have to meet a Leader of the Pack, the wolf with yellow glowing eyes. If you have an infant or a toddler in the house or have a wooden area your chance will be higher, wolves are somehow attached to babies.
Or you can just use the " boolProp testingCheatsEnabled true " cheat, then "shift+click" on the mailbox, click on "make NPC"(or something like that), "more...", "leader of the pack". Both the daily and lifetime relationship bars have to be above 50for the wolf to nibble your sim.
You don't always get the same wolf when you do this, my sim now knows like 10 leader of the pack's. It can take 20 minutes to an hour in sim time for the wolf to appear.

You should meet the wolf with yellow eyes and have a good relationship with his.
