or is this something that has to been done through the courts or an attorney. I am sick and will be going throug chemo so I am going to let my 13 year old daughter go back to Wv with parents until I am recovered. Hopefully only about 6 months...but the school there requires gaurdianship papers. thanks

Try www.findforms.com. If that doesn't work out, I would try contacting a notary in your area. The papers would probably have to be notarized anyway and the notary should be able to write something up for you that would be legally binding.

Go to your local law library and get the forms they are free and you need to get them notarized

I dont think you can download them but go to the department of human services childs division and they can give you the paperwork you need. ( thats the same place you apply for food stamps, welfare etc.)
