Well, my friend take heart there are many, not only in Tamilnadu, but also in Kerala, Karnataka and A.P, where Kovanam, or konakam, or langotti, is traditionally worn. The Kalaripayattu, the traditional form of Martial arts of kerala, insists on students to wear kovanam even now. When you wear langotti, it is drawn under your legs twice, thus your testicles are drawn up and tightly kept, thus it is possible to move your legs perpendicularly upwards and the efforts will be hardly felt by your testicles. It is not only effective, but also cost effective. It is again very durable and can be worn in any climate. No wonder the farmers of South India used it as the modern men use Bermuda.

Wearing Langot as an undergarment is not a proper thing medically. As it is worn very tightly, the testicles are kept in a hot atmosphere, which is harmful for the sperm count (in fertile males, off course). Testicle is a very tender part of body of males, therefore nature has put them outside and in a hanging position, so that it will be kept in airy and cool temperature. In Maharashtra, only Pahilwans (wrestlers) wear Langot,

Traditionally, Almost all men might have worn this undergarment about 40- 50 years ago before this VIPs arrived. Coming to your question..following it on this century .. might be ... who had started in their child days. Medically , people say it might be good or bad , I don't think it has any -eve impact. personally, I wore it till college and gave to up change to modern ones..
