6) Shared Bandwidth. If you're on a Satellite or Cable connection, you're sharing bandwidth with others (like your neighbors). If there's too many people using the same connection at the same time, the connection speed for everybody slows down (this is similar to issue #4).
7) Incorrect Bandwidth Capping. While this is highly unlikely, it's possible that your ISP incorrectly configured your broadband modem & you're getting stiffed with a slower connection.

Besides issues #5 & 7, there's nothing much you can do about it.
For Issue #5, it can be remedied by securing your WiFi router through various ways (WEP / WPA encryption, MAC filtering & disabling SSID broadcasting).
For Issue #7, you'll probably want to check with your ISP to ensure that the bandwidth caps are properly set.
For Issue #4, you could try to control how others use the internet connection... but they'll most likely blow you off unless you can control the router(s).

Yes, I have seen alot of different issues when it comes to slow/not working internet connections. Most of the time it is spyware. This can easliy be fixed by downloading Spybot Search & Destory from www.safer-networking.org and http://www.microsoft.com/athome/security/spyware/software/default.mspx from Microsoft. Both of these programs are free and do a good job cleaning. However I recommend you use both because sometimes one will miss some spyware while the other picks it up.
Another issue I have seen is when users expect megabytes when it is megabits. See there is a differnece in bytes and bits. It takes 8 bits(digits) to equal 1 byte. So as you can see megabits would be slower speed then megabytes. Usually you can tell by the abbreviations for example MB stands for Megabytes cause it is capitalize and mb stands for megabits.
Again there could be something that is slowing your connection down through the network. Where the wires are causing loops and data errors.
I think you should call your ISP.

Have you gone over your download limit and been slowed down as a consequence? Defrag etc won't help your net speed...
Do you have a wireless router and someone is stealing your connection?
It might just be some sites that are on a slow server...
Your best bet would to call your ISP and explain your problem...
The problem being a virus or spyware would be remote unless there is an unauthorized program continuously sending large packets of data away...

This may be due to some spywares or due to some trojans that are not dtected in normal virus scans. Try using AVG Virus scanner for scanning. If possible do an online scan at trendmicro.com. For detecting spywares try using Ewido Spyware scanner. I think this will sort out the problem.

If you are on at a time of day when most others are, then it can be like the dorm showers . . . low pressure when everybody is in there. Have you tried it in the off hours ? Is it still slow then ? and is it slow at all sites or just some ?

If it doesnt run like i suppose to be and afte those manitance
Check on your firewall sometimes it's usally the blocking
Check for files that you are Sharing aka Uploading
Check to see if you are on wireless and make sure it's seacure so that any one you don't know is not using.
fast way is,
start->run-> type in: cmd->press run
type in: net view
you should see only your computer name in there and the ones you know.
to exit command promopt type in EXIT :)
if all fails. it's your Interenet, they something like to screw you over by make your interent slow. they can adjuect the speed from their central, usally what they would tell you is that during that time something must have happend :).
I was with QWEST DSL, OMG they did to me all the time. to my neighbor and friends.
5mb d/l plan we where only getting 500k :(

perhaps your ISP has oversold their capacity. who is your ISP? capacity can be oversold at many points within their network. or it could be some other issue. without giving specific traceroute or ping it is impossible to answer your question.
