For instance, I was recently watching the Megan Mullaly show and author Mitch Albom was on promoting his new book. All of the audience members recieved the book and I was just wondering, did the publisher pay for those books or did the Megan Mullaly show itself pay for the books for the audience?

Material items are usually provided free of charge because to be televised on a talk show is pretty much free advertising, just as talking to a movie star about their upcoming film is free advertising for the movie to promote the item. Occasionally items from the Oprah show are paid by her company (trips, etc) but most are provided by the maker/publisher.

The manufacturer of the product pays for the giveaways. It's free air time (i.e. commercial).
