Yes-- It is highly unlikely, but still possible. For example, did you know that if a man wears a condom during sex, his partner still has a 2-4% chance of getting pregnant? So the best way to avoid getting pregnant is to be on the pill and use protection, and do whatever other methods there are to minimize the chances.

Even if you take them properly and never miss a single one, you can get pregnant. It is unlikely, but I know someone who had 3 kids before she was 19, and was on a different kind of birth control every single time.

Yes, this is very possible. Nothing is fool proof. I know someone who missed just one, cause' she and her boyfriend were out of town, and it was Memorial day, and the stores were closed! Bingo, just like that.

I think I've heard you're chances are higher then, because of the hormones that are in the pill.

Condoms and a barrier method might help. Not sure about the the barrier method, but anything with nonoxenol 9 explodes condoms!

Not if you take them every day at the same time and don't miss any. I took them for 6.5 years and never got pregnant, I quit them to get pregnant and a month later I was pregnant.
