You should go in for a medical check up. Explain your situation to your doctor and they will probably send you for testing. This situation you describe can come from a few different areas:
1. Cysts on your ovaries as in poly cystic ovarian disease
2. Sometimes the adrenal glands
3. Sometimes something to do with the pituitary
4. Some women just have more androgen's/DHT (male hormone) in their body produced, especially if you are under stress, with no factors that can be found.
It can be treated with an anti androgen medication such as aldactone can help with the hair loss and the acne. Possibly some birth control pills can help. However, if you want to conceive you cannot take this medication while trying or while being pregnant. It would be harmful to a male fetus. If you are overweight, losing weight can/will help.
Some people find help using saw palmetto capsules used by men for their prostates. Saw palmetto has anti androgen properties in it. It is a natural herb. Also mens Rogaine for the hair loss as it will also block the effects of DHT. Again both of these are not good if you are trying to conceive children.
The anti androgen medications start to work usually around 3 months, give or take some time. If you are also getting a thicker "mustache" than usual or hair where you do not want it, the medication would help with that too. Good Health and Good Luck. :)
P.s. have you recently come off any medication like birth control pills? They could cause a hormoen fluctuation. If it does not clear up, visit a doctor!

There are other things that can cause hair loss
1) stress
2) dieting ( not geting enough vitamins and nutrients in your diet which comes mostly from dieting)
3) etc.
Your probably having a sudden surge of hormones which is suddenly causing the acne, Sometimes we can be just fine and all of a sudden we get out of whack... You can try Proactive for the acne.. some think its a little pricey but well worth the results that you will get..
You can also start on a multivitamin. One that is high in vitamin E. A health food store would be the best place because they sell things that are more natural and better for you.
Good Luck

Have you recently tried to lose a lot of weight? Sometimes when your body experiences a traumatic change such as weight loss, it will react in such a way.
If not it may also be a thyroid problem. Thyroid problems are hereditary, so check with your family and see if anyone else has had the same problem. More importantnly check with your physican, this seems very serious!

the androgenic (male) hormones can cause both hair loss and acne. you don't say if you have any other symptoms of masculinization, but they could include reducing breast size, deepening of the voice, and hair growth in places that are inappropriate for a female, such as facial hair, chest, back or stomach. it seems like it would be worth seeing a doctor, because even if the problem is not hormonal, they may be able to help you. good luck.

Sounds like hormone problems, you may have polycystic ovary disease, which has, among its symptoms the ones you describe.
Check with your gynecologist.
