Yes. Someone died on the roller coaster at the West Edmonton Mall when it jumped the tracks. You would think a mall would have better saftey checks than that. Imagine how bad the equipment is, then, on those travelling exhibitions.

It is possible. Maybe that particular person has some kind of disease like w/e heart attack, it is possible for them to die on a roller coaster.

You can die anywhere. I have known there be fatal accidents with roller coasters - especially in Blackpool where I live.

depending on the situation yes, most theme rides are completely safe, but there have been some circumstances where deaths have accrued. it doesn't usually happen though, so don't worry about it.
