if its somthing i can do in 30 seconds at my table then yeah, if it takes more of my time then i dont unless somthing was wrong

If I have the time sure, after all, if it would help improve a restaurant - why wouldn't you want to see improvement. After all, how do they know what kind of job they are doing if they don't get any input.

Yes from Burger Kings receipts which entitles me to free whopper next time I go there. On their receipt is 800#, take 3 min for their questionaire, and they (recording) gives you a number to write on receipt.
