Botulinum toxin, produced by strains of C. botulinum is the most toxic natural compound exist on earth. About 20-30 nano gram of it would kill a full-grown adult.

As for element... any "heavy" radioactive elements with very short half life (e.g. man-made), such as Americium or Californium would be very lethal to living being.

Within reach??? It depend on how far you are trying to reach those things... Botulinum toxin for example is used in botox therapy or C. botulinum can be found almost everywhere, such as in bad canned products (and that's teh reason why you DON'T and SHOULDN"T eat bad canned stuff).

Botulinum is regarded as the most toxic substance on earth. But it is really "within reach"? If you leave food spoil, perhaps, but it will never be concentrated and pure.
For pure element, plutonium is regarded as deadly toxic, but again, it is not something that you should find in your neighborhood.

That will depend on what you consider within reach.
For elements and compounds in the household I will say ethanol, bleach, lye, mercury, prescription drugs and cigarettes.
