You should have the right to smoke on your lunch or dinner hour, provided you leave the building. You have no right to smoke while you are being paid, but if you are on an unpaid lunch break, they can't stop you from leaving the building and smoking. If you are allowed to leave the workplace during a break, you should be able to smoke, if not, too bad. Why should you be given special breaks to go out and smoke while non-smokers are stuck inside working?
As far as having to wear 2 1/2" heels and not be allowed to sit in a chair, you may have a better case. If you have a physical problem standing for so long and/or wearing heels, you may have a case under the Americans With Disabilities act. If you have a disability, you have a right to reasonable accommodation of your problem, which may include a chair and not having to wear high heels. I would suggest contacting your state's labor department and see what they have to say.

At my job in a hospital, day shift can leave only on their lunch breaks for a ciggarette (which is 7 hours after we've been at work) evening shift can leave on their break for a smoke too (that's after we've been there for 3 hours) Here's the kicker though: if you work nights or 12 hour night shifts you are NOT allowed to leave the building for any break and no smoking is allowed on campus. We've taken it up with a lawyer and there's nothing that can be done about it. In fact if we leave during the night hours we will be charged with abandonment and it goes against our licenses.

Look up the ACAS site online for more info. It doesnt sound fair to me. If you have a break where you are not paid, then you are entitled to do as you like - go out with no shoes and have a couple of cigies.
To keep you standing for 8 hours in high heels is out of order, get onto ACAS and your union if you have one.

Get some legal advice about this. I can't see how they can keep you from leaving the building and not letting you sit down whilst making you wear heels isn't right. if you don't get any help on this i suggest changing jobs.
