Once science allows humans to do body and tissue transplants, to replace the worn down body parts (hearts, lungs, bones). Could the human mind (brain) live and last forever if the body around it was just replaced as needed?

Yes actually it is possible. Certain cells in the body only regenerate a specific # of times. Those cells tend to be what determines "natural death". Cells are capable of regenerating infinite numbers of times, and some do (like epithelial cells). If all of the cells of the body could be reprogrammed so that they didn't "count" when they regenerate, then yes it is possible to live forever. This is a question several institutions are beginning to work on, and several others have been working on since the 1970s.

We don't know! Some say we already live for ever, but not trapped inside this body. If we were able to prolong life indefinitely, would out brains keep up? No one knows.

we were not meant to live in this body forever but in the afterlife we will be made perfect [and i might actually be able to spell and stuff] in this afterlife we will live forever with God if we have come to know Christ as our savior or in Hell if not and pay for our sins ourselves. its all in the Bible don't take my word for it, read it yourselves or "ask" about the Romans road

no entropy will cause everything to wear down. your internal organs would fail and your brain would eventually die.

the only way you can achieve eternal life is by asking Jesus Christ to save you and bring you to live with him forever in heaven

The body will be good but the brain will age....brain cell die and are not replaced.....daily.
